The Department for Medical Information and Records – Ein Kerem and Mount Scopus are engaged in collecting, maintaining, reviewing and creating copies of medical records that are produced at the Hadassah University Medical Center, with strict attention to maintaining medical confidentiality subject to the Patient’s Rights Law. All medical records are collected and scanned into the system in order to provide continuous, updated and accessible medical treatment. In addition, we are involved in coding medical diagnoses in accordance with the international ICD9 code and reporting to the Ministry of Health in all medical fields.

You can receive medical documents including:

  • Copies of illness summary/discharge letter
  • Surgical reports
  • Pathology reports
  • Laboratory test results and other tests
  • Complete file

How to contact us?

Attached is an online form or you can send a request by email. Clearly state which information you are requesting and attach a photocopy of your identity card. Priority will be given to requests made using the online form.
For your information, Ministry of Health procedures stipulate that receipt of copies of medical records are subject to payment of a fee. The information will be sent by secure email or Israel Post within one month.

Request for information through a representative:

  • If the patient is represented by a proxy, you must send the power of attorney notice from the patient to the person requesting the service, with a photocopy of the identity card of the patient and the proxy.
  • The patient’s natural guardian (for example, a parent of a child under the age of 18) may send a request with a photocopy of the identity card’s additional section.
  • Requests for patients who are not legally competent must be accompanied by a photocopy of the guardianship certificate and a photocopy of the identity card.

Request from an attorney:

Attorneys should send their requests with regard to hospitalizations with a waiver of medical confidentiality in their favor by post or email.

Request for information for a deceased patient:

Medical records for a deceased patient will be sent solely to a first degree relative or heirs listed in an inheritance order/probate of will. If an inheritance order has not yet been issued, it is necessary to submit a form for heirs that is signed by an attorney and the heir who is requesting the documentation. The request must be sent with a photocopy of the identity card as well.

Request for medical information for further treatment:

In order to receive information for the purpose of further treatment, contact the attending physician who will send an official request. The information will be sent directly to the physician. Please note that this is not a request to receive a complete hospitalization file. This information does not require payment of a fee.
